good faith estimate
you have a right to receive a "Good Faith Estimate" to let you know how much your treatment cost will be. this is done to prevent surprise medical billing. you will be presented with a Good Faith Estimate prior to your first appointment and we will discuss further in our first session.
I am out of network with insurance and do not accept insurance which means your insurance will not cover your expenses with me directly. you can submit a claim to your insurance personally after I provide you with a requested superbill.

superbill
while I do not bill your insurance, if you wish to provide a superbill to your insurance to seek reimbursement for treatment, please provide a request for a superbill. I then will provide you a superbill of the services rendered for you to submit to your insurance.
each insurance is different and may have out-of-network benefits in which they will reimburse for some healthcare services. call your insurance provider to ask for more information on your out-of-network benefits.
